For the Department of Defense schools, Hyalite is a registered contractor in the U.S. Federal Government’s Central Contractor Registration (CCR) database (active status).

 

About the Author

The author, Dr. Mark D. Nelson, has been a school counselor and a school counselor educator for the past 19 years. During his career, Dr. Nelson has delivered more than 1,000 hours of classroom guidance activities using experiential learning techniques. He has also designed, delivered, and evaluated school counseling programs at both local and state levels.

 

General Information

Each publication includes about 100 classroom activities that are organized and designed to meet the National Standards created by the American School Counselor Association (ASCA). These activities can be used in the design of a comprehensive guidance curriculum, which addresses the academic, career, and personal/social development of all students.

 

Accountability is becoming an essential ingredient to school counseling programs throughout the nation. Each of The Guides provide assessment instruments that allow counselors to measure how school counseling programs make a difference for their students. These assessment instruments measure the guidance curriculum's effectiveness to promote the academic, career, and personal/social developmental domains. These publications have been assembled in three-ring binders in order to make it convenient for the user.

 

Using a Purchase Order

Any order that uses a purchase order for payment must have a billing address of a school, district, or state department of education office located within the United States. A properly authorized original or complete copy of the P.O. is required and must be received before an order can be processed. If the delivery address on the purchase order and the order form are different, the order will be shipped to the address on the purchase order. (For international orders, please refer to the section below for ordering and payment methods.)

 

Order Form

An order form for these publications is available as a printer-friendly Adobe PDF file, which can either be printed and completed manually or completed electronically. All fields except the credit card number can be filled in online; therefore, you will need to print the partially completed form before manually entering the credit card number.

 

Please mail your completed order form and payment (or purchase order) to Hyalite, LLC, P.O. Box 5084, Bozeman, MT 59717-5084 or fax them to (406) 282-7726 or (866) 840-5135 (toll-free). If you do not receive your order or a shipment confirmation within 2 weeks, please contact us to make sure that we received your order.

 

Payment

The total cost of each publication is $150.00, which includes standard shipping (see below). Sales tax for orders to  all U.S. destinations is not applicable since they are shipped from Montana, which does not have a sales tax.

 

Payment terms are net 30 days for school purchase orders. Personal orders must be accompanied by a check, money order, or credit card information (we accept Visa and MasterCard). All payments must be made in U.S. funds and drawn on a U.S. institution. (If applicable, please refer to the section below regarding international shipments.)

 

Shipping & Handling

Domestic Shipments. Standard shipping and handling charges for destinations within the United States are free. All orders will be shipped either via the U.S. Postal Service or FedEx Ground. Orders are usually shipped the following business day after receipt of your order; however, please allow up to ten days for shipping. If you provide your e-mail address on the order form, you can elect to be notified when your order has been shipped. If you have not received your order or a shipment confirmation within 10 days, please contact us to confirm our receipt of your order.

 

Shipping charges for Express Mail will be based on the actual cost plus a $5.00 expediting fee per order. We must receive your order by noon Mountain Time in order to accommodate rush orders. The Express Mail charges are a minimum of $28.75 for 1 book, $38.15 for 2 books, and $47.20 for 3 books. However, due to variations in weight and destination, the exact charges vary; so please contact us for an accurate quote prior to submitting your rush order. Also, please be advised that delivery may take longer than 1 day, based upon the drop-off time and delivery address.

 

International Shipments. All international orders must be pre-paid via credit card (Visa or MasterCard) or check drawn on a U.S. bank. Once your payment has been received and processed, we will ship your order. Since the shipping charges vary by the weight of the order and the shipping destination, we will notify you via email as to the available shipping method options, costs, and estimated delivery times. You may choose which delivery option that you prefer. After you notify us of your shipping option and submit your payment, including shipping costs, we will process and ship your order. All custom fees, import duties, and/or taxes, if any, will be extra and will be the responsibility of the buyer at the time of delivery. Packages refused at the time of delivery will not be refunded.

 

We guarantee that packages sent to international destinations are delivered to and accepted by the delivery service (either the U.S. Postal Service or FedEx) at the point of departure. After which, however, we are not able to take responsibility for the delivery of the shipment to you.

 

Preview and Desk Copies

Publications are not available for preview (no exceptions). Complimentary copies are also not available. However, notices will be posted on this web site as to the times and places that these publications will be exhibited. Also, please check out the sample pages below.

 

Return Policy

No refunds, returns, or exchanges will be made. Copies of The Guide will be replaced only when an order arrives damaged. All our orders are properly packaged for safe delivery; however, should your order arrive damaged, please contact us within 48 hours for a replacement.

 

Cancellations

Cancellations must occur prior to the shipping of your order. Orders are typically shipped the morning of the following business day.

 

Copying

All of our publications are copyrighted and may not be copied. The following exception applies however:

  • Handouts (pages on which the word "Handout" is included in the page header) may be reproduced in quantities sufficient for distribution to students in those classrooms utilizing The Guide. All other reproduction, for any purpose is strictly prohibited.
